Telista – MCL 50 (Strip of 10 Tablets)

Composition (per film-coated tablet):

  1. Metoprolol (50 mg):
    • Metoprolol is a beta-blocker that works by blocking beta-1 adrenergic receptors in the heart. This helps to reduce the heart rate, lower blood pressure, and decrease the heart’s workload. It is used to treat hypertension (high blood pressure), angina (chest pain), and heart failure.
    • Metoprolol also helps prevent arrhythmias (irregular heartbeats) and is often prescribed after heart attacks to reduce the risk of subsequent cardiac events.
  2. Cilnidipine (10 mg):
    • Cilnidipine is a calcium channel blocker (CCB), specifically a dual-acting calcium channel blocker. It blocks both L-type and T-type calcium channels in the vascular smooth muscle and heart, leading to vasodilation (widening of blood vessels) and reduction in blood pressure.
    • Unlike other calcium channel blockers, Cilnidipine has the unique ability to reduce sympathetic nervous system activity, which makes it particularly effective for hypertension and diabetic kidney disease. It also has less effect on heart rate compared to other calcium channel blockers.
  3. Telmisartan (40 mg):
    • Telmisartan is an angiotensin II receptor blocker (ARB), which works by blocking the action of angiotensin II, a hormone that narrows blood vessels. This causes the blood vessels to relax and widen, which in turn lowers blood pressure and reduces strain on the heart.
    • Telmisartan is effective in reducing the risk of heart attacks, stroke, and kidney complications, particularly in people with diabetes or chronic kidney disease.

Pharmacological Action:

  • Metoprolol slows the heart rate and reduces the force of heart contractions, which lowers blood pressure and helps reduce the oxygen demand of the heart. It also helps prevent arrhythmias and protects the heart after a heart attack.
  • Cilnidipine relaxes the blood vessels by blocking calcium entry into the smooth muscle cells of the vascular system, causing vasodilation and lowering blood pressure. Its ability to act on both L-type and T-type calcium channels makes it unique in treating hypertension with fewer side effects related to reflex tachycardia (rapid heart rate).
  • Telmisartan reduces blood pressure by blocking the effects of angiotensin II on its receptor, causing vasodilation and helping prevent the constriction of blood vessels. This medication is also beneficial for kidney protection in patients with hypertension or diabetes.

Indications:

Telista – MCL 50 is used to treat:

  1. Hypertension (High Blood Pressure): The combination of Metoprolol, Cilnidipine, and Telmisartan works synergistically to lower blood pressure effectively.
  2. Angina (Chest Pain): Metoprolol helps reduce chest pain and prevents further episodes by lowering the heart’s workload.
  3. Heart Failure: Metoprolol helps improve heart function by reducing heart rate and the strain on the heart. Telmisartan contributes to protecting the heart by lowering blood pressure.
  4. Diabetic Nephropathy (Kidney Damage due to Diabetes): Telmisartan helps protect kidney function in patients with diabetes, and Cilnidipine can reduce kidney damage by controlling high blood pressure.

Dosage:

  • Usual Dose: One tablet of Telista – MCL 50 is taken once daily. The combination of Metoprolol (50 mg), Cilnidipine (10 mg), and Telmisartan (40 mg) allows for effective blood pressure management in a single dose.
  • The dose may be adjusted depending on the patient’s blood pressure and health condition. If blood pressure remains uncontrolled, a healthcare provider may increase the dose or add additional medications.
  • Telista – MCL 50 can be taken with or without food.

Side Effects:

Common side effects may include:

  • Dizziness or lightheadedness (due to lowered blood pressure).
  • Fatigue and tiredness.
  • Headache.
  • Swelling (edema), especially in the lower extremities, due to Cilnidipine.
  • Slow heart rate (bradycardia), particularly from Metoprolol.
  • Gastrointestinal discomfort (nausea, abdominal pain).
  • Cough or angioedema (swelling of the lips, face, or throat) due to Telmisartan.

Serious side effects may include:

  • Severe dizziness or fainting due to low blood pressure.
  • Heart-related issues: Bradycardia or arrhythmias, especially with Metoprolol.
  • Kidney issues: Kidney dysfunction may occur, particularly in patients with pre-existing kidney conditions.
  • Severe allergic reactions (e.g., swelling of the face or difficulty breathing).

Precautions:

  • Pregnancy: Telmisartan should not be used during pregnancy, especially in the second and third trimesters, as it may harm the fetus. Metoprolol and Cilnidipine are also contraindicated in pregnancy unless absolutely necessary.
  • Breastfeeding: Telmisartan and Metoprolol may pass into breast milk, so it is best to avoid their use during breastfeeding or consult a healthcare provider for alternatives.
  • Kidney or Liver Disease: Patients with kidney or liver issues should use Telista – MCL 50 with caution, as all three components (Metoprolol, Cilnidipine, Telmisartan) require dose adjustments or monitoring in these conditions.

  • Heart Issues: Metoprolol should be used cautiously in patients with heart block or bradycardia.

  • Store Telista – MCL 50 tablets in a cool, dry place, below 25°C.
  • Keep out of the reach of children.
  • Do not use after the expiry date.
