Composition (per film-coated tablet):
- Telmisartan I.P (40 mg):
- Telmisartan is an angiotensin II receptor blocker (ARB), primarily used to manage hypertension (high blood pressure). It works by blocking the effects of angiotensin II, a hormone that causes blood vessels to constrict. By preventing its action, Telmisartan helps relax and dilate the blood vessels, which lowers blood pressure and reduces the workload on the heart.
- Telmisartan also helps protect the kidneys in patients with conditions like diabetic nephropathy (kidney damage due to diabetes).
- Amlodipine Besilate I.P (5 mg):
- Amlodipine is a calcium channel blocker (CCB) that works by relaxing and widening the blood vessels, which makes it easier for the heart to pump blood. Amlodipine specifically blocks calcium from entering the smooth muscle cells of the blood vessels and the heart, which helps reduce blood pressure and improve blood flow.
- Amlodipine is commonly used to treat hypertension and angina (chest pain). It also helps prevent conditions like heart attacks and strokes by improving circulation and reducing the heart’s workload.
Pharmacological Action:
- Telmisartan: By blocking the angiotensin II receptor, Telmisartan prevents the vasoconstrictive effects of angiotensin II, leading to vasodilation (relaxation of blood vessels) and lower blood pressure. It also protects the kidneys in high-risk patients.
- Amlodipine: As a calcium channel blocker, Amlodipine inhibits calcium ion influx into the smooth muscle cells, which relaxes blood vessels and lowers blood pressure. It also helps reduce the workload on the heart by preventing excessive contraction of heart muscle cells.
Indications:
Telista-AM is used to treat:
- Hypertension (High Blood Pressure): The combination of Telmisartan and Amlodipine effectively lowers blood pressure, with the additional benefit of improving cardiovascular health.
- Angina (Chest Pain): Amlodipine works by relaxing blood vessels, improving blood flow, and reducing angina (chest pain).
- Risk Reduction in Cardiovascular Diseases: The combination of both drugs provides comprehensive protection against conditions such as heart attack, stroke, and kidney damage, particularly in patients with diabetes.
Dosage:
- Usual Dose: One Telista-AM tablet once daily. The dose may be adjusted based on the severity of hypertension and individual response.
- The typical starting dose is 40 mg of Telmisartan and 5 mg of Amlodipine. However, the dosage can be adjusted by the prescribing doctor depending on the patient’s blood pressure and overall health condition.
- Amlodipine is a long-acting drug, so it provides 24-hour control with just a once-daily dose.
Side Effects:
Common side effects include:
- Dizziness or lightheadedness (due to blood pressure reduction).
- Swelling (especially in the legs, ankles, or feet), which may occur due to Amlodipine.
- Fatigue.
- Headache.
- Flushing or warmth in the skin.
- Gastrointestinal issues: such as nausea or stomach pain.
- Elevated potassium levels (hyperkalemia): due to Telmisartan.
- Low blood pressure (hypotension), especially after the first dose.
Serious side effects may include:
- Severe allergic reactions (e.g., swelling of the face, lips, or throat, difficulty breathing).
- Kidney dysfunction: Monitor kidney function in patients at risk.
- Heart problems: Such as a very slow heart rate (bradycardia) or heart failure.
Precautions:
- Pregnancy and Breastfeeding: Telmisartan should not be used during pregnancy, particularly in the second and third trimesters, as it may harm the fetus. It is also not recommended during breastfeeding.
- Kidney and Liver Function: Caution is needed for people with kidney or liver problems. Telmisartan and Amlodipine may require dosage adjustments, and kidney function should be monitored regularly.
- Low Blood Pressure: Patients with low blood pressure or those who are volume-depleted (e.g., from dehydration or diuretics) should use this medication with caution.
- Electrolyte Imbalance: Regular monitoring of potassium levels is advised, as Telmisartan may cause elevated potassium levels (hyperkalemia).
- Heart Issues: Amlodipine can sometimes cause a slow heart rate (bradycardia) or heart failure, so it must be used with caution in patients with pre-existing heart conditions.
Storage:
- Store Telista-AM tablets in a cool, dry place at temperatures below 25°C.
- Keep out of reach of children.
- Do not use the tablets past their expiry date.
