Composition (per tablet):

• Telmisartan IP (40 mg):
  • Telmisartan is an angiotensin II receptor blocker (ARB). It works by blocking the action of angiotensin II, a hormone that narrows blood vessels. By inhibiting this hormone, Telmisartan helps relax the blood vessels, which in turn lowers blood pressure. This makes it effective in treating high blood pressure (hypertension).
  • It also helps reduce the workload on the heart, which can be beneficial for people with heart failure or those at risk for cardiovascular events.

Pharmacological Action:

• Telmisartan selectively blocks the angiotensin II type 1 (AT1) receptor, preventing angiotensin II from exerting its effects of vasoconstriction (narrowing of blood vessels), aldosterone release (which causes sodium retention and increased blood volume), and sympathetic activation.
• By blocking these effects, Telmisartan helps in:
  • Lowering blood pressure (antihypertensive effect).
  • Reducing the risk of stroke and heart attack in patients with high blood pressure.
  • Protecting kidney function in patients with diabetes or renal disease.

Indications:

Telista 40 is used to treat the following conditions:

1. Hypertension (High Blood Pressure): Helps to lower and control blood pressure, thereby reducing the risk of stroke, heart attack, and kidney problems.
2. Cardiovascular Risk Reduction: In patients who are at risk for cardiovascular events, Telmisartan can help reduce the risk of major cardiovascular events, such as heart attack or stroke.
3. Chronic Kidney Disease: Often used in patients with chronic kidney disease, especially those with diabetes, to protect kidney function.

Dosage:

• Initial Dose: The usual starting dose for Telmisartan in adults is 40 mg once daily.
• Maintenance Dose: Based on the patient’s response, the dose may be increased to 80 mg once daily.
• Maximum Dose: 80 mg once daily is typically the maximum dose.

Telmisartan can be taken with or without food, but it is important to take it at the same time each day for consistency in blood pressure control.

Side Effects:

Common side effects may include:

• Dizziness or lightheadedness: Especially when standing up quickly, due to its blood pressure-lowering effect.
• Headache.
• Fatigue.
• Gastrointestinal issues: Such as nausea, diarrhea, or abdominal pain.
• Elevated blood potassium levels (hyperkalemia), which can be dangerous if not monitored.
• Low blood pressure (hypotension): Especially in patients who are volume-depleted (e.g., those on diuretics).

Serious side effects are rare but can include:

• Angioedema (swelling of the face, lips, tongue, or throat).
• Kidney dysfunction: Impaired kidney function can occur, especially in those with pre-existing kidney disease or dehydration.

Precautions:

• Pregnancy and Breastfeeding: Telmisartan should not be used during pregnancy (especially in the second and third trimesters) as it may harm the fetus. It is also not recommended for breastfeeding.
• Kidney or Liver Issues: Patients with impaired kidney or liver function should use Telmisartan with caution. Regular monitoring of kidney function is advised.
• Electrolyte Imbalance: Check for hyperkalemia (high potassium levels) and monitor for low blood pressure.
• Dehydration: Caution is required in patients who are dehydrated, such as those on diuretics or with diarrhea or vomiting.

Storage:

• Store Telista 40 tablets in a cool, dry place below 25°C.
• Keep the tablets out of the reach of children.
• Do not use the medication past its expiry date.
