Sitared-MD XR 1000 is an extended-release combination medication used for the management of type 2 diabetes mellitus. It contains two active ingredients: Sitagliptin and Metformin, both of which are commonly used to help control blood sugar levels in people with diabetes.
Active Ingredients:
- Sitagliptin (1000 mg):
- Sitagliptin is a DPP-4 inhibitor (Dipeptidyl Peptidase-4 inhibitor). It works by increasing the levels of incretins—hormones that help increase insulin production after meals and decrease the amount of sugar produced by the liver.
- Sitagliptin helps regulate blood sugar by enhancing insulin secretion when blood sugar is elevated and reducing the liver’s production of glucose.
- Metformin (1000 mg):
- Metformin is a biguanide that primarily works by decreasing hepatic glucose production (reducing the amount of glucose produced by the liver) and increasing insulin sensitivity in muscle and fat tissues. This leads to improved glucose uptake by the cells and helps lower blood sugar levels.
- Metformin is typically used as a first-line treatment for type 2 diabetes.
Formulation: Extended-Release (XR)
- The “XR” in Sitared-MD XR indicates that the tablet is extended-release. This formulation allows the medication to be released gradually over time, providing a sustained effect and helping to maintain more consistent blood sugar levels throughout the day.
- The extended-release version may be associated with fewer gastrointestinal side effects (such as nausea or diarrhea) compared to the immediate-release formulations of Metformin.
Indications:
Sitared-MD XR 1000 is used for:
- Type 2 diabetes mellitus: It is prescribed to help control blood sugar levels in adults with type 2 diabetes. This medication is often used when diet, exercise, and other medications do not sufficiently control blood sugar.
- Improvement of glycemic control: The combination of Sitagliptin and Metformin helps improve overall blood sugar control and reduces the risk of long-term complications of diabetes.
Dosage:
- The usual starting dose of Sitared-MD XR 1000 is one tablet (1000 mg of Sitagliptin and 1000 mg of Metformin) once a day with food to reduce gastrointestinal discomfort.
- Depending on the individual’s response and blood sugar levels, the dosage may be adjusted by the healthcare provider.
- Extended-release tablets should be taken whole and not crushed or broken, as it may affect the release of the medication.
Side Effects:
Sitared-MD XR 1000 may cause side effects, though not everyone experiences them. Common and serious side effects include:
- Common side effects:
- Gastrointestinal issues: Nausea, vomiting, stomach pain, diarrhea, or loss of appetite (more common with Metformin).
- Headache.
- Upper respiratory infections or sore throat.
- Fatigue or dizziness.
- Serious side effects (rare but require immediate medical attention):
- Lactic acidosis: A rare but serious condition, more common in patients with kidney problems, characterized by symptoms like muscle pain, difficulty breathing, weakness, or feeling cold.
- Hypoglycemia (low blood sugar): This can occur when Sitared-MD XR is used in combination with other antidiabetic medications that stimulate insulin production (e.g., insulin or sulfonylureas).
- Pancreatitis: Although rare, Sitagliptin has been associated with pancreatitis (inflammation of the pancreas), which may present as severe abdominal pain.
- Allergic reactions: Rash, itching, or swelling (especially in the face or throat), which may indicate an allergic reaction.

Precautions:

Warnings:
- Lactic acidosis: This is a serious and potentially life-threatening condition that can occur, though it is rare. Symptoms include muscle pain or cramping, unusual tiredness, difficulty breathing, and feeling cold or dizzy. If you experience these symptoms, stop taking the medication and seek immediate medical help.
- Hypoglycemia: While Sitagliptin is unlikely to cause hypoglycemia on its own, it may increase the risk when used with other medications that stimulate insulin release.
- Kidney function: Regular monitoring of kidney function is important during treatment, as Metformin can accumulate in the body in cases of kidney dysfunction, leading to increased risk of side effects, including lactic acidosis.
