Sitared M 50/1000 is a combination medication used for the management of type 2 diabetes mellitus. It contains Sitagliptin and Metformin, two widely used medications that work together to control blood sugar levels.

Active Ingredients:

1. Sitagliptin (50 mg):
   • Sitagliptin is a DPP-4 inhibitor (Dipeptidyl Peptidase-4 inhibitor). It works by increasing the levels of incretins in the body. Incretins are hormones that stimulate insulin production when blood glucose levels are high and also inhibit glucose production by the liver.
   • Sitagliptin improves insulin secretion and helps in the regulation of blood sugar levels after meals.
2. Metformin (1000 mg):
   • Metformin is a biguanide that helps lower blood sugar by:
     • Reducing hepatic glucose production (the amount of glucose the liver makes).
     • Increasing insulin sensitivity in muscle and fat cells, allowing for better glucose uptake.
     • Decreasing intestinal glucose absorption.

Indications:

Sitared M 50/1000 is typically used for:

• Type 2 diabetes mellitus: It helps control blood sugar levels in individuals with type 2 diabetes, especially when diet and exercise alone do not sufficiently manage the condition.
• Glycemic control: It improves overall blood glucose control, helping to reduce complications associated with high blood sugar levels.

Dosage:

• The usual starting dose of Sitared M 50/1000 is 1 tablet (containing 50 mg Sitagliptin and 1000 mg Metformin) once a day, typically with a meal to minimize potential gastrointestinal side effects.
• The dosage may be adjusted by your doctor based on your blood sugar levels and tolerance to the medication.
• Sitared M 50/1000 is available in a strip of 15 tablets, and the number of tablets taken daily may depend on individual treatment needs.

Side Effects:

Sitared M 50/1000 may cause side effects, although not everyone experiences them. Common and serious side effects include:

• Common side effects:
  • Gastrointestinal issues: Nausea, vomiting, diarrhea, stomach discomfort, and loss of appetite (more common with Metformin).
  • Headache.
  • Dizziness or fatigue.
  • Upper respiratory infections or sore throat.
• Serious side effects (require immediate medical attention):
  • Lactic acidosis: A rare but serious condition associated with Metformin, characterized by muscle pain, weakness, difficulty breathing, unusual tiredness, and feeling cold.
  • Pancreatitis (inflammation of the pancreas) is a potential side effect of Sitagliptin, though it is rare. Symptoms include severe abdominal pain.
  • Hypoglycemia (low blood sugar): While Sitagliptin alone is less likely to cause hypoglycemia, the risk may increase when taken with other antidiabetic medications like insulin or sulfonylureas.
  • Allergic reactions: Rashes, swelling, or difficulty breathing may indicate an allergic reaction.

Precautions:

• Kidney or liver issues: Both Sitagliptin and Metformin are processed by the kidneys, and kidney function should be monitored regularly. Metformin is not recommended for patients with severe renal impairment. In such cases, Sitagliptin may also require dosage adjustments.
• Alcohol: Excessive alcohol consumption should be avoided as it increases the risk of lactic acidosis when taking Metformin.

• Hypoglycemia: The risk of low blood sugar is low with Sitared M 50/1000 unless combined with other medications that increase insulin levels. Symptoms of low blood sugar include shakiness, sweating, dizziness, and confusion.
• Monitoring kidney function: Regular testing of kidney function is recommended during treatment with Sitared M.
