Pulmocef-250 is a broad-spectrum oral antibiotic belonging to the second-generation cephalosporin group. It is widely prescribed for the treatment of bacterial infections affecting the respiratory tract, urinary tract, ear, nose and throat, skin, and soft tissues. The medicine is intended for use only under medical supervision.
Composition
Each film-coated tablet contains Cefuroxime Axetil IP equivalent to Cefuroxime 250 mg.
Other inactive ingredients are present in suitable quantities to maintain tablet stability and absorption.
Description
Cefuroxime Axetil is a prodrug that is converted in the body into its active form, Cefuroxime. After oral administration, it is absorbed from the gastrointestinal tract and rapidly hydrolysed to the active antibiotic. Pulmocef-250 is effective against a wide range of Gram-positive and Gram-negative bacteria.
Pharmacological Class
Pulmocef-250 belongs to the beta-lactam class of antibiotics and is categorised as a second-generation cephalosporin. It exhibits bactericidal activity.
Mechanism of Action
Cefuroxime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins present in the bacterial cell membrane, preventing the formation of a strong and protective cell wall. This leads to weakening of the bacterial structure, cell rupture, and eventual death of the bacteria.
Antibacterial Spectrum
Pulmocef-250 is active against many common pathogens including Streptococcus pneumoniae, Streptococcus pyogenes, Staphylococcus aureus (non-MRSA strains), Haemophilus influenzae, Escherichia coli, Klebsiella species, Proteus mirabilis, Moraxella catarrhalis, and Neisseria gonorrhoeae. It is not effective against MRSA, Pseudomonas aeruginosa, or Enterococcus species.
Pharmacokinetics
After oral administration, absorption is enhanced when the tablet is taken with food. The drug is widely distributed in body tissues including lungs, sinuses, tonsils, skin, urinary tract, and bones. Cefuroxime is primarily eliminated through the kidneys in unchanged form. The average half-life is approximately one to one and a half hours in individuals with normal renal function.
Therapeutic Indications
Pulmocef-250 is used in the treatment of respiratory tract infections such as bronchitis and pneumonia, ear and throat infections including tonsillitis and otitis media, sinusitis, urinary tract infections, skin and soft tissue infections, bone and joint infections, and certain sexually transmitted infections when prescribed by a physician.
Dosage and Administration
The usual adult dose is 250 mg taken twice daily. In more severe infections, the dose may be increased as directed by the physician. The duration of treatment generally ranges from seven to ten days depending on the severity of infection and clinical response. The tablet should be swallowed whole with water and taken after food for better absorption.
Use in Special Populations
In patients with kidney impairment, dose adjustment may be required. Elderly patients should be monitored for renal function. Use during pregnancy should be based on medical advice, and caution is advised during breastfeeding as small amounts of the drug may pass into breast milk.
Contraindications
Pulmocef-250 should not be used in patients with known hypersensitivity to cefuroxime, other cephalosporins, or beta-lactam antibiotics. It is also contraindicated in individuals with a history of severe allergic reactions to penicillins.
Warnings and Precautions
Caution is advised in patients with a history of gastrointestinal disorders, particularly colitis. Prolonged use may result in secondary infections. Allergic reactions, including severe skin reactions, may occur and require immediate medical attention. Renal function should be monitored during long-term therapy.
Drug Interactions
Pulmocef-250 may interact with medicines such as probenecid, anticoagulants, oral contraceptives, nephrotoxic drugs, and live bacterial vaccines. Patients should inform their doctor about all medicines they are taking.
Side Effects
Common side effects include nausea, vomiting, diarrhoea, abdominal discomfort, headache, and dizziness. Less common effects include skin rash, itching, and fungal infections. Rare but serious adverse reactions include severe allergic reactions, antibiotic-associated colitis, blood disorders, and serious skin conditions.
Overdose
Overdose symptoms may include severe gastrointestinal disturbances and, in rare cases, seizures. Immediate medical attention is required, and treatment is supportive.
Laboratory Test Interference
Pulmocef-250 may interfere with certain urine glucose tests and Coombs tests.
Storage
Store in a cool, dry place below 25°C. Protect from light and moisture. Keep out of reach of children.
