Dosage Form and Pharmaceutical Class

Midodrive 2.5 mg is an oral tablet belonging to the class of alpha-1 adrenergic agonists. It is primarily used to increase blood pressure in patients with symptomatic orthostatic hypotension.


Description

Midodrive tablets are designed for oral administration and provide a low-dose formulation (2.5 mg) suitable for titration in patients sensitive to blood pressure changes. The tablets are intended for systemic absorption, with predictable pharmacodynamic effects on vascular tone.


Mechanism of Action

Midodrine is a prodrug that is metabolized into desglymidodrine, an active metabolite. It stimulates alpha-1 adrenergic receptors in blood vessels, causing vasoconstriction. This increases peripheral vascular resistance and raises blood pressure, particularly when standing, thereby reducing symptoms of dizziness or fainting in orthostatic hypotension.


Pharmacological Properties

  • Absorption: Rapidly absorbed from the gastrointestinal tract.

  • Metabolism: Converted to active metabolite desglymidodrine by liver and kidney enzymes.

  • Elimination: Excreted primarily in urine.

  • Onset of Action: Typically within 30–60 minutes after oral dosing.

  • Duration of Effect: 2–4 hours per dose.


Therapeutic Indications

Midodrive 2.5 mg tablets are used for:

  • Symptomatic orthostatic hypotension caused by conditions such as:

    • Autonomic dysfunction

    • Neurodegenerative disorders (e.g., Parkinson’s disease)

    • Other causes of chronic low blood pressure

  • Helps reduce dizziness, lightheadedness, and fainting episodes associated with low standing blood pressure.


Dosage and Administration

  • Dosage must be individualized based on blood pressure response and symptom control.

  • Tablets are usually taken 2–3 times daily, with at least 4 hours between doses.

  • Avoid bedtime dosing to prevent supine hypertension.

  • Swallow the tablet whole with water; may be taken with or without food.


Contraindications

  • Known hypersensitivity to midodrine or any component of the formulation.

  • Severe organic heart disease, acute renal disease, or thyrotoxicosis may require caution.

  • Patients with supine hypertension should not take the medication at bedtime.


Warnings and Precautions

  • Monitor blood pressure: Supine hypertension can occur, especially with evening doses.

Drug Interactions

  • Antihypertensives: May counteract blood pressure-lowering medications.

  • Vasoconstrictors or sympathomimetics: May potentiate hypertensive effects.

  • MAO inhibitors or other adrenergic agents: Monitor closely to avoid excessive blood pressure elevation.


Use in Special Populations

  • Pregnancy: Only if clearly necessary.

  • Breastfeeding: Consult physician; limited data on excretion in milk.

  • Pediatric Use: Safety and efficacy in children not well established; use only if prescribed.


Overdose

Symptoms may include severe hypertension, headache, and bradycardia. Seek immediate medical attention if overdose is suspected. Treatment is supportive, including monitoring blood pressure and cardiovascular function.


Missed Dose

Take the missed dose as soon as remembered if it is not near the next scheduled dose. Do not double the dose. Avoid taking doses close to bedtime.


Storage

Store in a cool, dry place away from sunlight, moisture, and heat. Keep out of reach of children.

  • Monitor blood pressure, especially when lying down.

  • Avoid taking the tablet just before sleep to reduce risk of supine hypertension.
