Generic Name: Bisoprolol Fumarate
Strength: 5 mg
Dosage Form: Film-coated tablet
Drug Class: Cardio-selective Beta-1 Adrenergic Blocker

🔹 INDICATIONS / USES

Used in the management of:

1. Hypertension (High BP)

2. Chronic stable angina

3. Chronic heart failure (with reduced ejection fraction)

4. Tachycardia / Palpitations

5. Post-myocardial infarction (as cardioprotection – doctor advised)

🔹 MECHANISM OF ACTION (How it works)

Bisoprolol:

• Blocks β1-receptors in the heart

• ↓ Heart rate

• ↓ Cardiac output

• ↓ Renin release from kidneys
  ➡️ Leads to lower blood pressure and reduced heart workload

It has minimal effect on lungs compared to non-selective beta blockers.

🔹 DOSAGE & ADMINISTRATION

• Usual dose: 2.5–10 mg once daily

• 5 mg is a common maintenance dose

• Take once daily in the morning

• Can be taken with or without food

• Swallow whole with water

⚠️ Do not crush or chew

🔹 ONSET & DURATION

• Onset of action: 1–2 hours

• Peak effect: 3–4 hours

• Duration: 24 hours

🔹 COMMON SIDE EFFECTS

Usually mild and dose-dependent:

• Fatigue / weakness

• Dizziness

• Slow heart rate

• Cold hands & feet

• Headache

• Nausea

🔹 RARE BUT SERIOUS SIDE EFFECTS

• Severe bradycardia

• Heart block

• Worsening heart failure

• Bronchospasm (rare)

➡️ Seek medical help if:

• Very slow pulse (<50/min)

• Fainting

• Severe breathlessness

🔹 CONTRAINDICATIONS (DO NOT USE IN)

• Severe bradycardia

• 2nd or 3rd degree heart block

• Cardiogenic shock

• Severe hypotension

• Untreated heart failure

🔹 USE WITH CAUTION IN

• Diabetes (masks hypoglycemia symptoms)

• Asthma / COPD

• Thyroid disorders

• Elderly patients

• Liver or kidney disease

🔹 DRUG INTERACTIONS

Avoid or monitor with:

• Verapamil, Diltiazem

• Digoxin

• Amiodarone

• Other antihypertensives

• Insulin & oral antidiabetics

🔹 IMPORTANT PATIENT COUNSELING POINTS

✔ Take daily at the same time
✔ Check BP and pulse regularly
✔ Never stop suddenly (risk of rebound hypertension/angina)
✔ If a dose is missed, take it the same day (do not double dose)

• Pregnancy: Use only if clearly needed

• Breastfeeding: Use with caution (passes into milk)
